No physical harm is actually being done to the target but the mental stress it places on its victims (basically the feeling they're dying/in agony) can kill/immobilize/etc. There are limitations like the link above suggests, it doesn't work on those who are intoxicated, it can be reflected back (i.e through mirror or something), require eye contact, etc.
